Is it cache that helps increase your DPS as bear when using incarn?

On the first boss how did the monk die? I thought he dodged Vy's blast? Is that what he died from?
Also, I think one reason that you got Vy was the shaman's vesper totem kind of looks like it was near it. Vesper does 114% more damage to the closest target and it actually looks like it died from a vesper blast.
